Coaching Agreement

Client Name:
This agreement between coach, _________, and the client will begin
upon the first payment (made ________) and will continue for a period of _____ or for X number of sessions.

Fees
The fee for this coaching package is $77/session (1hr. or can be split into 2 sessions at 30 minutes). These fees will be paid in advance.

IF APPLICABLE - This is a package for a period of ___ weeks including ____ bi-weekly appointments for ____ minutes at a time.

Additional appointments can be scheduled as needed.

Commitment
Coaching is an ongoing relationship between a Coach and a Client. The opportunity for success for the coaching client dramatically increases through changing behavioral habits and creating new possibilities. This is a process.

Coaching is a structure that facilitates the process of personal, professional, and/or spiritual development. The Client and Coach agree that the coaching relationship will be designed together.

By entering this relationship, the Coach and Client acknowledge that the Client wants to make significant progress and change in his/her life. Because progress and change happen at rates that are unique to each individual, the Coach and Client commit to working with each other for an initial three‐month period. This allows the coaching relationship necessary time to develop and progress through objectives, obstacles, and successes that occur.
Coaching is for individuals who are emotionally and psychologically healthy and who want to make positive changes in their lives. Coaching is not advice, therapy, or counseling. By entering this relationship, the Coach and Client acknowledge that the Client wants to make significant progress and change in his/her life. Progress and change happen at rates that are unique to each individual. By entering into this agreement, the client accepts full responsibility for implementing suggestions and completing any assignments in a timely manner.

Services
The services to be provided by the Coach to the Client are face-to-face or telephone/virtual coaching, as agreed jointly with the Client. Coaching may address specific personal projects, business successes, or general conditions in the client's life or profession. Other coaching services include value clarification, brainstorming, identifying plans of action, examining modes of operating in life, asking clarifying questions, and making empowering requests or suggestions for action.

Throughout the working relationship, the Coach will engage in direct and personal conversations. The Client understands that successful coaching requires a co-active collaborative approach between Client and Coach. In the coaching relationship, the Coach plays the role of a facilitator of change, but it is the Client's responsibility to enact or bring about the change.

By entering into this agreement, the Client accepts full responsibility for implementing suggestions and completing any assignments in a timely manner. Failure to complete assignments in a timely manner may result in a suspension of calls and or coaching until the assignment has been completed.

If the Client believes the coaching is not working as desired, the Client will communicate and take action to return the power to the coaching relationship. This may entail reevaluating goals, a restructuring of the coaching relationship, or a termination of the relationship.

Coaching Sessions
Coaching sessions are conducted by telephone or Skype unless otherwise arranged with the Coach. At the scheduled time, the Coach agrees to call the Client at an agreed upon number. The Coach and Client commit to start and finish each session on time. If the Client is more than 10 minutes late for a coaching session, the Coach will assume the session is canceled and the Client will forfeit the session time and fee.
Rescheduling a coaching session is easily done with appropriate notice. Please allow for at least 24 hours notice to appointment changes. In between coaching sessions, the Client may
feel free to e‐mail the Coach (please limit emails to 1 per week). Calls made between the scheduled calls are permissible when the Coach has availability; the parties agree to keep the calls to 5 minutes or less.

Prior History
The Client also agrees to disclose details of the past or present psychological or psychiatric treatment. Coaching, as defined earlier in the contract, is not a substitute for counseling, therapy, and other forms of psychiatric, mental, or physical care.
I do not engage in therapy with my coaching clients. In entering into the coaching relationship, and signing the agreement, you are agreeing that if any mental health difficulties arise during the course of the coaching relationship, you will notify me immediately so that our work can be suspended while you receive proper care.

Privacy
The Client can, at any point in the coaching session, declare his/her preference not to discuss a specific issue, by simply stating that they would rather not discuss this issue. The Coach agrees to respect this boundary and will not attempt to forward the conversation further along those lines.

Confidentiality
The Coach will work within the professional ethics and guidelines as designated by the International Coaching Federation (see www.coachfederation.org). Copies of the ethical guidelines are available on the website. All information about the Coach/Client relationship will remain strictly confidential except in very rare circumstances where decreed by law; ie. where the court might issue a subpoena for the file or information.

If you wish for me as your Coach to speak to someone outside our interactions, then you need to give me written permission (original letter, fax or email) to do so. Exceptions to confidentiality, of course, relate to circumstances such as intent to seriously harm someone, child abuse etc. Otherwise, all your information is confidential.

During the course of the coaching relationship, we may use various forms of technology to communicate and collaborate. The Client acknowledges that there are always risks associated with using certain media such as the Internet, mobile phones, and cordless phones. If you use these to communicate and collaborate with me, then I will assume that it is appropriate to continue to do so in my interactions with you.
Terms and Conditions
This coaching agreement is offered to you conditioned on your acceptance without modification of the terms, conditions, and notices contained on essentialhustle.com. Your use of essentialhustle.com, including the purchase of any products, services, or coaching constitutes your agreement to all such terms including, but not limited to, refund policies, intellectual property rights, and liability disclaimers. Please read these terms carefully, and keep a copy of them for you reference.

Termination
The Coach and Client agree to provide each other with two week’s notice in the event that it is desired to terminate coaching. Otherwise, the coaching will continue for the duration of the contracted period.

In the circumstance that the Client is on a payment plan, and has not fully paid at the time of their termination, they will still be required to pay the remainder of their contracted time, even if they decide not to utilize that time.
